Reports are extremely important in funding our Youth Action Centers.  Please submit your reports in a timely manner.

Project Summary Reports  - Download and complete a project summary after each project, and submit it to 21st Century Leaders. These reports should be completed within a week of the project and be accompanied by project sign-in sheets (below) and any pictures taken at the project. (After clicking, please "Save" this file)

Project Sign-In Sheets - These sheets are mandatory for each project executed by the Youth Action Center.  Please download this form and have it present at every YAC project.  This document will serve as required documentation for all students who participate in your service activities and should accompany ALL of your project summary reports. Rosters & Meeting Sign-In Sheets - The YAC Roster will document all of the student YAC members who regularly attend meeting and plan projects. This form is particularly important for our ability to to track the ongoing YAC participants. The Meeting Sign-In sheets are for the advisors to track student attendance to weekly meetings AND to ensure that students are actively participating in their YAC. Mid-year Report - due December 15, 2010

End-of-the-Year Report - due May 18, 2011
